The Referral Process

  • A referral form is completed and returned to us. This can be obtained by telephone, facsimile or email request. It can also be downloaded (see download section)
  • If the applicant meets the eligibility criteria they are invited to meet with the supported living team to find out more about the services we offer and to complete a needs assessment.
  • If the applicant is suitable for supported living then an action plan is drawn up.
  • If the applicant is not suitable for supported living then we will endeavour to refer them to a more appropriate service.
